Presentation: Improving HIV Health Outcomes from a Person-Centered Approach

Equitas Health’s medical clinic in the Short North (Columbus, OH) has been serving people living with HIV for over 5 years. This workshop will provide information on how patients have accessed care over time and what factors have contributed to predictable patterns of visits and what has possibly contributed to poorer access to care. Data will include distinctions by race, ethnicity, gender identity, orientation and other characteristics of the human experience. Presenters will invite participants to explore existing HIV performance measures in relation to social determinants of health. This presentation will also examine the necessity of identifying and designing outcome measures that align with population health to reduce health disparities and increase health promotion.
1. Review of the HRSA HIV/AIDS Bureau Performance Measures and the HIV Cascade.
2. Learn about visit patterns of adults living with HIV who are served in an urban community-based health center over a number of years.
3. Gain an understanding of the relationship between social determinants of health, health care access and health outcomes.
4. Explore how the design of person-centered health outcomes can benefit people living with HIV and reduce health disparities.
